End of life care support

As a nursing assistant at the Hospice in the Weald I provided hands on support to clients in their final stages of life, assisting with all personal care of their choice or according to their own personal needs, whilst also monitoring symptoms and ensuring comfort at all times. I reported any changes in their condition to medical staff on duty or if in the family home, I would have access to a response team.  Emotional support is key, offering companionship and comfort to both clients and their families. My role extended to End of life care, ensuring clients dignity, peace and comfort in their final moments.                                  

I was also at hand to provide support to families, answering questions and offering reassurance during a difficult time.

Dementia Care

As a Healthcare assistant in Dementia care, I supported Clients through various stages of the condition.  I assisted them with daily tasks such as Personal care and dressing, sometimes with encouraging my Client to eat and assisting when they had difficulty, all whilst maintaining personal hygiene.  Managed challenging behaviours ensuring a calm and safe environment, while helping clients remain as independent as possible.  Cognitive stimulation through activities and social interaction is crucial.  Monitored changes in mood, memory and their behaviour, reporting these to Healthcare professionals.               

Additionally, I provided emotional support to both my Clients and their families offering re-assurance and guidance in managing the symptoms and challenges associated with dementia at different stages.

Motor Neuron Disease

Helping someone with motor neuron disease {MND} and respiratory problems involves managing breathing and daily care needs. My role was to support respiratory function by helping with non-invasive ventilation {NIV} devices, clearing mucus using suction or assisted coughing, always ensuring equipment is properly fitted.  I assisted with daily tasks such as moving, personal care, in some cases assisted with feeding, repositioning my Client for personal and breathing comfort.  

Monitoring symptoms like breathlessness and fatigue is crucial, with Healthcare assistants alerting medical staff if issues arise.  I was able to administer medication for symptom relief under supervision.                                 

Emotional support is key, as respiratory issues can cause anxiety, As assistants we offer reassurance and keep communication between the care team and family.

My Services

Night Care Services

For those needing assistance overnight, I provide trusted and attentive care to ensure peace of mind throughout the night. Services include:

  • Sleeping Night Care – I remain on-site and available if needed, offering reassurance through the night.

  • Waking Night Care – I provide active support all night, assisting with mobility, medication reminders, toileting, or other needs.

Extended Care Visits (Minimum 3 Hours Per Visit)

I offer longer care visits with a minimum duration of 3 hours per visit, allowing time for thorough, unhurried, and personalised support. Services include:

  • Personal care and hygiene assistance

  • Support with mobility and daily activities

  • Companionship and emotional support

  • Meal preparation and light household tasks
